Incident Reporting

If an incident occurs on a trip, please follow the procedure outlined within the UQ Sport trip plan for your trip.

 1. UQ Sport Incident Reporting
Incident report should be done within 24hrs if possible.

Please use the UQ sport incident reporting form available online. This is also part of the insurance process.

The online UQ sport incident reporting form is the primary reporting mechanism. However if you are remote this may not be possible. There are paper reporting forms in the club first aid kits as a backup until a connection becomes available.

2. Alert the Club Committee
The online form goes directly to UQ sport. The club committee does not get a copy. Please also alert the club committee ASAP by email and/or phone if there are incidents or hazards identified. The committee may need to take actions in response, and is also keen to monitor the outcome of any incidents.

3. Paddle Australia Incident Report Form
The UQ Sport reporting forms are generic for all clubs. They do not record information specific for canoeing. In addition to the primary UQ Sport reporting form, the club will request that a paddle specific form is completed for any serious incident. This is a paper form from Paddle Australia and is also located in the club first aid kits.
